【Application】
Step one: submit application to your local long-term care center;
Step two: receive home assessment by experts;
Step three: make a care plan together.
Approved applicants are entitled to use the services, including home services, day care, pickup and drop off, respite services, etc.
【Eligibility】
# those who are assessed and determined by experts to be:
- dysfunctional and aged 65 and over;
- dysfunctional, aboriginal and aged 55 and over;
- dysfunctional and mentally or physically challenged;
- suffering from dementia and aged 50 and over;
- elderly who lives alone and requires only assistance in instrumental activities of daily living
- weak and vulnerable elderly who requires only assistance in instrumental activities of daily living
